  • Visit our website and select Client Portal from our menu.
  • In the client portal select request appointment and then select an appointment date and time that would meet your clients needs. (TIP: Appointments within 24 hours require a call to the office, while appointments beyond 24 hours do not).
  • Select "continue without signing in" and omplete the short form with the client information. If you would also like an e-mail confirmation that the appointment was accepted, please request this in the message to practice box..
  • Fax supporting documents to our secure fax (848) 244-9343.
  • We will respond within a few hours to conform the appointment.
